Frequently Asked Questions

 

What should I bring with me to my first appointment? 

Please bring the following to your appointment: 

* Any films, scans or reports that have been taken for your condition. It is your responsibility to bring these to your first appointment so that you can be examined and information reviewed at the same time. If you do not have the actual scans, your appointment will need to be rescheduled. 

* Insurance card 

* Exact copay amount 

* Referral or authorization from your primary care physician to see a specialist. If your insurance requires a referral or authorization from your primary care physician to see a specialist, then it is your responsibility to obtain that referral before you come to your office visit. If you do not have this referral, then our office will be required to reschedule your appointment. You should contact your insurance company directly to verify whether or not you require a referral. 

* Picture identification 

* New patient registration forms (download below). Please fill out completely and either fax in to our office at 713-756-8305 or mail to us at 1213 Hermann Drive, Suite 675, Houston, Texas 77004. The originals of your forms should be brought to your appointment.

What insurances is the practice participating with?

Since there are many divisions of insurance companies, it would be in your best interest to call your insurance provider to see if Elizabeth Bonefas, M.D. or Concepcion Diaz - Arrastia M.D. is an in-network provider.

When can I get my test results?

Dr. Bonefas or Dr. Diaz-Arrastia will go through your test results at your follow-up or post operative appointment.  However if you would like, you can call the office within five business days of the test and ask the medical assistant for your results. If the results are not back yet, the medical assistant will give you further instructions as to the availability of the results

When can I take my dressing off?

How do I get my prescription refilled?

Prescription refills should be called in during regular business hours.  If it is after hours, you will be asked to leave a message so that this can be called in during the next business day.

What is considered an emergency?

If you have increased swelling and redness, severe pain or fever greater than 101 degrees for 24 hours, please contact our office immediately at 713-756-8555.

How do I schedule tests, surgeries or a mammogram?

Please call our office at 713-756-8555.  Or email procedurescheduling@womenshealthsurgeons.com to schedule any of these items. 

What are some of the risk factors that might occur after breast surgery?

Some of the risk factors that could occur after breast surgery are:  infection, bleeding, poor scarring, seroma (fluid collection), hematoma (blood collection) and loss of skin.

Does the practice charge a fee for filling out various forms?

The office charges a fee for filling out medical forms such as those for FMLA and Disability.  Please call the office to check on the cost and list of forms that require a fee.
